BLOOD PRESSURE SELF MONITORING PROGRAM
TAKE ACTION TO IMPROVE HEART HEALTH
According to the CDC, nearly half of all American adults have high blood pressure, and only 1/4 have their condition under control.
The simple process of checking and recording your blood pressure at least twice a month over a period of four months has been shown to lower blood pressure in some people with hypertension.
Blood Pressure Self-Monitoring is a free evidence-based program that features personalized support for developing a blood pressure self-monitoring routine, tips for maintaining cardiovascular health and provides nutrition education seminars. This program also includes a complimentary YMCA membership for the duration of the program.
Through the four-month program and with the support from a trained staff member, participants in this program will aim to:
- Measure and record their blood pressure at home at least two times per month, gaining increased awareness of triggers that elevate blood pressure 
- Attend two personalized consultations per month with a Healthy Heart Ambassador 
- Attend monthly Nutrition Education Seminars 
PROGRAM GOALS
- Reduction in blood pressure 
- Better blood pressure management 
- Increased awareness of triggers that elevate blood pressure 
- Enhanced knowledge to develop healthier eating habits 
WHO QUALIFIES
- Be at least 18 years old 
- Be diagnosed with high blood pressure or currently taking antihypertensive medication 
- Not have experienced a recent cardiac event 
- Not have atrial fibrillation or other arrhythmias 
- Not be at risk for lymphedema 
YMCA membership is not required.
